Patient Collaborative Care Terms (Collab Care™)

Welcome to Collab Care™, the Patient Collaborative Care Program by Silver House!

This is a policy of Silver House Healthcare LLC (“we,” “us,” “our,” or “Silver House”) and applies to its Collaborative Care Communities, which include but are not limited to MemoryWatch™, Circles™, and BATWatch™. These communities are collectively or individually referred to as (our “Community,” “MemoryWatch,” “Circles,” or “BATWatch”).

Each Collaborative Care Community has been specifically developed to target distinct stages and aspects of cognitive health management:

  • MemoryWatch™ is a program that focuses on supporting individuals and families impacted by cognitive decline or dementia, providing access to a vast network of healthcare providers, assisted living facilities (ALFs), and support systems tailored to these needs.
  • Circles™ is a program that serves those in the pre-clinical stages of cognitive decline, offering social, lifestyle, and wellness activities aimed at promoting cognitive health early in life to prevent chronic stress and long-term decline.
  • BATWatch™ is a program designed to empower healthcare providers by offering resources and collaborative frameworks to support early detection and management of Alzheimer’s through BAT Levels™ monitoring, testing, and treatment.

By enrolling or participating in any of these communities, all Members agree to abide by the terms outlined herein and any specific community-based guidelines related to their care or services.

Below are the Terms regarding your participation in Collab Care™, a Patient Collaborative Care Program (the “Program”). This program consists of specialty medical services focused on supporting your cognitive health at every stage—whether you’re taking preventive measures or managing a diagnosed condition.

We leverage the full range of tools, resources, and expertise within each community to create a care environment tailored to each individual’s unique needs. Whether through advanced monitoring, social engagement, or preventative care, our communities are equipped to meet almost every medical need related to cognitive health.

We are committed to customizing the Collaborative Care experience based on the individual needs of our patients. By joining one of our communities, patients benefit from a highly adaptable care model that integrates medical services, personal support, and cutting-edge treatments designed to improve outcomes and enhance the quality of life.

By focusing on personalized cognitive health monitoring at all stages, the Program helps reduce unnecessary hospital visits, improves management of cognitive risks, and enhances overall well-being—whether you are receiving care in-home or in assisted living settings. Through Telehealth services, remote monitoring, and collaborative care management, we ensure that your healthcare team is always informed and equipped to support you.

  1. Silver House’s Collaborative Approach

Our approach to Collaborative Care is grounded in the belief that healthcare works best when it’s built on strong partnerships between patients, families, and healthcare professionals. Each of our Collaborative Care Communities is designed not only to deliver medical services but to create a network of care that leverages expertise from multiple sources—physicians, caregivers, specialists, and support systems.

We believe that every patient deserves a care plan that adapts to their evolving needs. That’s why we’ve integrated specialized tools, resources, and services into our approach to enhance patient care. From BATWatch™’s early detection frameworks to MemoryWatch™’s cognitive health management, each community is equipped with the resources necessary to provide comprehensive support, no matter where you are in your cognitive health journey.

  1. Core Components of the Program

Our collaborative approach doesn’t stop at just providing care; it’s about continuous engagement and proactive management of health. These services may include but are not limited to:

  • Personalized Treatment Plans: Every patient receives a care plan that’s customized to their unique needs and risks, ensuring the highest quality of care.
  • Remote Monitoring & Telehealth: Our advanced telehealth services and remote monitoring allow us to track your health progress in real-time, reducing the need for in-person visits while still ensuring high-quality care.
  • Collaborative Decision-Making: We foster a team-based approach to care, where your healthcare providers collaborate seamlessly to keep you on track, with regular updates on your health status.
  • Telehealth Services: Remote consultations with healthcare providers to address cognitive health concerns and develop personalized care plans.
  • Remote Patient Monitoring (RPM): Use of monitoring devices to track health indicators related to cognitive function, including vital signs and activity levels.
  • Remote Therapeutic Monitoring (RTM): Monitoring of therapeutic progress through self-reports and clinical interventions to ensure that cognitive health treatments are effective.
  • Collaborative Care Management (CoCM): Coordination between healthcare providers to ensure that cognitive care plans are aligned with a patient’s broader medical needs.
  • Primary Care Management (PCM): A holistic approach to managing a patient’s overall care needs, with a particular focus on cognitive health.
  • Chronic Care Management (CCM): Ongoing support for patients dealing with chronic cognitive health issues, ensuring continuity of care.
  • Behavioral Health Integration (BHI): Services that address the mental health aspects of cognitive decline, integrating behavioral health into the overall care plan.
  • CogStim™ Cognitive Therapy: Personalized cognitive therapies, including speech-language therapy, physical therapy, and behavioral interventions, aimed at improving or maintaining cognitive function.

By integrating technology, real-time data, and continuous collaboration, we create a dynamic care environment that evolves with your needs, ensuring that all aspects of your cognitive health are being addressed effectively. Our approach empowers you to take an active role in your healthcare, supported by a community that’s always there to guide and assist.

  1. How the Program Works

The Collaborative Care Program supports cognitive health across every stage—from early prevention to managing later stages of cognitive decline. By utilizing cutting-edge diagnostics, therapeutic interventions, and community-centered activities, we help patients maintain cognitive function and improve their quality of life at each phase. Below is a breakdown of how each of our programs works:

3.1 Early Prevention with Circles™

Circles™ is designed for individuals in the pre-clinical stage of cognitive decline, typically for those in their 20s to 40s. The focus here is on preventative care, which integrates social, lifestyle, and wellness activities to reduce factors like isolation and chronic stress that contribute to cognitive impairment.

Patients can engage in activities such as:

  • Mindfulness and meditation workshops aimed at stress reduction.
  • Exercise programs tailored to support mental health, such as yoga or walking groups.
  • Cognitive wellness education, including nutritional counseling to encourage brain-healthy diets (e.g., Mediterranean or MIND diets).
  • Community-based events such as book clubs, social gatherings, and volunteer programs, which foster engagement and mental stimulation.

These preventive measures are designed to promote cognitive resilience early in life, laying a foundation for long-term brain health.

3.2 Midlife Cognitive Health with BATWatch™

For individuals in their 40s and 50s, BATWatch™ focuses on early detection of Alzheimer’s disease and other cognitive conditions through the use of advanced diagnostics like the BAT Test™, which measures beta-amyloid and tau protein levels to assess risk.

In addition to diagnostics, patients with elevated BAT levels can access BAT Pill treatment protocols, which are designed to manage and lower these harmful protein levels. This cutting-edge treatment targets the reduction of beta-amyloid and tau buildup, helping to prevent or slow the onset of cognitive symptoms.

BATWatch™ also includes:

  • Regular screening and follow-up care to track changes in beta-amyloid and tau levels.
  • Lifestyle modification counseling, which focuses on diet, exercise, and mental activities proven to reduce cognitive risks.

These tools allow for proactive intervention, helping patients address potential cognitive decline before symptoms develop.

3.3 Managing Cognitive Decline with MemoryWatch™

For individuals experiencing cognitive decline or dementia, MemoryWatch™ provides a comprehensive suite of services aimed at managing and slowing the progression of cognitive disorders. Central to this program is CogStim™ Cognitive Therapy, which offers personalized, evidence-based interventions that engage the brain and body.

Key CogStim™ interventions include:

  • Speech-language therapy: Focused on maintaining communication skills and cognitive-linguistic function.
  • Physical therapy: Designed to improve motor function and mobility, which supports cognitive health by enhancing blood flow and engagement in daily activities.
  • Occupational therapy: Helps patients maintain independence by improving their ability to perform everyday tasks.
  • Behavioral health support: Offers mental health counseling and stress management to alleviate emotional distress caused by cognitive decline.
  • Cognitive stimulation exercises: Engaging the brain in mental tasks like puzzles, memory games, or art therapy to help improve memory retention and cognitive flexibility.

In addition, Remote Patient Monitoring (RPM) and Remote Therapeutic Monitoring (RTM) allow for continuous tracking of health metrics, such as blood pressure, sleep patterns, and physical activity, ensuring that the care team can quickly respond to any changes in the patient’s condition.

By combining real-time data with personalized care plans, MemoryWatch™ provides patients with the tools they need to navigate cognitive decline while improving their quality of life.

  1. Eligibility and Conditions for Participation

The Program is designed for individuals at all stages of cognitive health—whether proactively managing cognitive wellness or dealing with advanced impairments in assisted living facilities (ALFs) or home care settings. The program’s primary objectives include:

* Proactive Cognitive Health Management: The Program emphasizes early detection and continuous care, helping to slow or stall cognitive decline by addressing potential risks before they become critical.
* Comprehensive Support Across Cognitive Conditions:
The Program caters to individuals at any stage of cognitive risk, whether mild cognitive impairment (MCI) or advanced dementia, ensuring their needs are met with tailored care strategies.
* Minimizing Hospitalizations Through Personalized Care Plans
By offering personalized, comprehensive care plans, the Program reduces the need for hospital visits and acute care interventions. It ensures a higher standard of health management through regular monitoring and proactive support that anticipates potential health crises before they escalate.

  1. Voluntary Participation and Assumption of Inherent Risk

Participation in the Program ™ and its associated programs, including MemoryWatch™, Circles™, BATWatch™, and any other cognitive health initiatives, is entirely voluntary and undertaken at the Participant’s own risk. While Silver House takes extensive precautions to ensure the safety and well-being of all participants, it is essential to acknowledge that cognitive health interventions and activities inherently involve risks that cannot be fully eliminated. These risks include, but are not limited to:

* Physical Therapy (PT): Risks such as muscle soreness, fatigue, falls, overexertion, or physical injury during therapeutic exercises.
* Occupational Therapy (OT): Risks including muscle strain, balance issues, falls, overexertion, emotional stress, or fatigue during daily living and functional activities.
* Speech-Language Therapy (SLP): Risks including choking, fatigue, emotional discomfort, or frustration during speech and swallowing exercises.
* Mental Health and Psychotherapy: Risks associated with emotional discomfort, psychological distress, and temporary feelings of vulnerability when discussing sensitive topics or working through mental health challenges.
* Behavioral Health and Behavior Modification: Risks including behavioral resistance, emotional reactions, or stress due to changes in routine, habits, or interactions, particularly in participants with cognitive or behavioral challenges.
* Remote Therapeutic Monitoring (RTM) and Remote Patient Monitoring (RPM): While designed for convenience, these remote services carry potential risks related to miscommunications, data transmission delays, or technical malfunctions, which may affect timely medical intervention.
* Community-Based Activities (Circles™): Risks inherent in group activities or physical events, such as physical strain, fatigue, or accidents. Additionally, social gatherings may pose emotional stress or discomfort for participants dealing with cognitive challenges, anxiety, or social isolation.
* Transportation to Events: Risks including traffic accidents, physical discomfort, or injuries while being transported to and from the Program or Circles™ events, whether transportation is provided by the participant or arranged by Circles Hosts.
* Cognitive Stimulation Activities (CogStim™): While focused on improving cognitive function, these activities may result in fatigue, frustration, or temporary cognitive stress as participants engage in challenging exercises or mental tasks.
* Medical Testing (BATWatch™): Risks including potential discomfort, minor pain, or emotional stress related to diagnostic procedures such as blood draws or lab tests.

  1. Our Commitment to Safety

Our primary commitment is the health and well-being of every participant. Silver House takes great care to tailor all interventions and activities to the specific needs and conditions of each individual, particularly those with cognitive impairments or chronic health conditions.

However, we emphasize that no program or intervention is entirely without risk, and each participant must engage with a full understanding of the potential risks involved.

  1. Release of Liability:You, and on behalf of your spouse, children, parents, guardians, heirs, next of kin, and any legal or personal representatives, executors, administrators, successors and assigns, or anyone else who might claim or sue on your behalf, (“Releasing Parties”) agree that by participating in the Program, hereby release Silver House, and its directors, officers, employees, agents, contractors, insurers, spectators, co-participants, equipment suppliers, and volunteers; all Community Members, Venue and their directors, officers, employees, agents, contractors, insurers, equipment suppliers, and volunteers; all by Silver House sponsors, organizers, promoters, directors, officials, and property owners; governmental bodies and/or municipal agencies whose property and/or personnel are used; and any or all parent, predecessor, subsidiary or affiliate companies, licensees, officers, directors, partners, board members, supervisors, insurers, agents, equipment suppliers, and representatives of any of the foregoing, (“Released Parties”), from any claims, demands, liabilities, rights, and causes of action of any kind, arising from or related to your participation in the Program, acknowledging the distinct nature of each and the informed consent Patient has provided for participation. This release includes, but is not limited to: Individual Variability, Diagnosis Specifics, Treatment History, Adherence to Therapy, Complex Needs, Personal Factors, Muscle Soreness and Fatigue, Increased Risk of Falls, Injury Risks, Aggravation of Pre-existing Conditions, Cognitive Challenges Affecting Compliance and Safety, Adverse Reactions to Exercise, Emotional Distress, Unpredictable Incidents, Muscle Strain and Overexertion, Falls and Balance Issues, Fatigue, Emotional Stress, Aggravation of Pre-existing Conditions, Adverse Reactions to Adaptive Equipment, Choking or Aspiration Risks, Fatigue, Frustration and Emotional Distress, Emotional Discomfort, Adverse Reaction to Augmentative and Alternative Communication (AAC) Devices, Psychological Distress, Changes in Behavior, Reaction to Medication, Adjustment to Therapy, Behavioral Health, Psychology, and Behavior Modification, Behavioral Resistance, Emotional Reactions, Adjustment Period, Interpersonal Dynamics, Unintended Consequences.

And General Risks include, but are not limited to:

Risks associated with the physical location and venue, including accidents, falls, and potential hazards related to the premises.
Risks related to environmental conditions, including outdoor activities subject to weather conditions, terrain, wildlife, and other natural elements.
Any other risks not foreseeable or mentioned herein, related to participation in the Program activities or presence at any Program session.
Injuries or damages resulting from undiagnosed or undisclosed health conditions.
This release covers all forms of liability, to the fullest extent permitted by law, including but not limited to, the ordinary negligence on the part of the Released Parties.

  1. Consent & Acknowledgements:

* Exclusive Provider Assignment: The Program can only be assigned to one medical provider at a time. Patients can still keep all existing care providers. This program can only have one collaborator, which work with Patient’s existing care teams seamlessly.
* Withdrawal Rights: Patient can withdraw from the Program or any parts of the Program, at any time by submitting a request.
* Participation in Care Models: Patient agrees to participate in PCM, CoCM, CCM, or BHI, which are essential components of PCCP, and engage with our Prime team for ongoing health management.
* Regular Monitoring: The Program may involve regular check-ups, screenings, and assessments to monitor Patient health status.
* Collaborative Care: Patient agrees to a collaborative care model, where Patient’s provider works closely with other healthcare professionals to ensure comprehensive care.
* Information Sharing: The Program may involve sharing relevant health information with Patient’s care team to facilitate coordination.

  1. Telehealth:

By engaging with the Program, You consent to the use of telehealth services. This includes the delivery of health care, advice, education, intervention, monitoring, and remote admissions through electronic communication and technology. You understand and agree with the following:

Telehealth/Telemedicine involves the use of electronic communications to enable health care providers at different locations to share individual patient medical information for the purpose of improving patient care.

Providers may include primary care practitioners, specialists and/or subspecialists, nurse practitioners, registered nurses, medical assistants, and other healthcare providers who are part of your clinical care team.

In addition to You and the members of your clinical care team, your family members, caregivers, or other legal representatives or guardians may join and participate on the telehealth/telemedicine service, and You agree to share your personal information with such family members, caregivers, legal representatives, or guardians. The information may be used for diagnosis, therapy, follow-up and/or education.

Telehealth/Telemedicine requires transmission, via Internet or tele-communication device, of health information, which may include:
Progress reports, assessments, or other intervention-related documents
Bio-physiological data transmitted electronically.
Videos, pictures, text messages, audio, and any digital form of data
The laws that protect the privacy and confidentiality of health and care information also apply to telehealth/telemedicine. Information obtained during telehealth/telemedicine that identifies me will not be given to anyone without your consent except for the purposes of treatment, education, billing, and healthcare operations.

By agreeing to use the telehealth/telemedicine services, You are consenting to Silver House sharing of your protected health information with certain third parties as more fully described in the Privacy Policy. I understand, agree, and expressly consent to Silver House obtaining, using, storing, and disseminating to necessary third parties, information about

You, including your image, as necessary to provide the telehealth/telemedicine services.

As with any Internet-based communication, You understand that there is a risk of security breach. Electronic systems used will incorporate network and software security protocols to protect the confidentiality of patient identification and imaging data and will include measures to safeguard the data and to ensure its integrity against intentional or unintentional corruption.

Individuals other than your clinical care team or consulting providers may also be present and have access to my information for the Telehealth/telemedicine session. This is so they can operate or repair the video or audio equipment used. These persons will adhere to applicable privacy and security policies.

Telehealth/telemedicine sessions may not always be possible. Disruptions of signals or problems with the Internet’s infrastructure may cause broadcast and reception problems (e.g., poor picture or sound quality, dropped connections, audio interference) that prevent effective interaction between consulting clinician(s), participant, patient, or care team.

You hereby release and hold harmless Silver House and all members of your care team from any loss of data or information due to technical failures associated with the telehealth/telemedicine service.

You understand and agree that the health information You provide at the time of your telehealth/telemedicine service may be the only source of health information used by the medical professionals during the course of your evaluation and treatment at the time of your telehealth/telemedicine visit, and that such professionals may not have access to your full medical record or information held with Silver House.

You understand that You will be given information about test(s), treatments(s) and procedures(s), as applicable, including the benefits, risks, possible problems or complications, and alternate choices for your medical care through the telehealth/telemedicine visit.

You have the right to withhold or withdraw consent to the use of telehealth/telemedicine services at any time and revert back to traditional in-person clinic services. You understand that if You withdraw my consent for telehealth/telemedicine, it will not affect any future services or care benefits to which You are entitled.

All your questions have been answered to your satisfaction.

You hereby consent to the use of telehealth/telemedicine in the provision of care and the above terms and conditions.

You certify that You are the legal representative of the participant or that You are the patient and are 18 years of age or older, or otherwise legally authorized to consent. You have carefully read and understand the above statements. You have had all Your questions answered. You understand that this informed consent will become a part of your medical record.

  1. Remote Patient Monitoring (RPM):

If you are setup with RPM, You must return any devices to Silver House if you withdraw from the Program.

You understand that the Program is not an emergency response program and is not monitored 24/7. Call 911 or go to ER for medical emergencies.

You are the only person who should be using any remote monitoring equipment as instructed. And will not use the device for reasons other than Your own personal health monitoring.

You will not tamper with any equipment.

You understand You are solely responsible for the safekeeping of the equipment and will be personally and financially responsible for any lost or damaged equipment.

You understand any devices are only designated for the Program and not designed for, intended, or permitted for use on the internet.

You understand that these devices may be used to collect weight, blood pressure readings, pulse and/or oxygen saturation levels, and transfer those readings to the Network Electronic Medical Record System.

You are aware Your daily readings from any device may be transmitted from the monitor and stored by Network in a safe and secure manner.

You are aware that Your device readings may be reviewed monthly, or periodically in accordance with the plan of care we have created for you, and NOT on a 24/7 basis. You will be contacted on a periodic basis, by phone, email, message, or Network app (whichever is most appropriate and agreed upon), to review and discuss my results and progress.

  1. Remote Therapeutic Monitoring (RTM):

If you are setup with RTM, You agree to actively engage with the RTM protocols, including providing accurate and timely self-reports, adhering to monitoring protocols, and participating in scheduled evaluations.

You understand that RTM involves self-reporting of relevant health information, including symptoms, medication adherence, and any changes in health status.

You agree to provide honest and complete self-reports during the monitoring period, as this information will be used to assess your health status and inform care decisions.

You acknowledge that the self-reports provided through RTM may contain personal health information and agree to the collection, transmission, and storage of such data for the purpose of monitoring and care coordination.

You understand that your privacy and data security will be protected in accordance with applicable laws and regulations, and that access to your health information will be limited to authorized healthcare providers.

You understand that regular assessments or updates may be provided to your care team based on the self-reports collected through RTM, and that this information will be used to inform your care plan and treatment decisions.

You are aware that Your reports may be reviewed monthly, or periodically in accordance with the plan of care we have created for you, and NOT on a 24/7 basis. You will be contacted on a periodic basis, by phone, email, message, or Network app (whichever is most appropriate and agreed upon), to review and discuss my results and progress.

  1. Privacy and Data Protection (HIPAA Compliance)

Your privacy is of the utmost importance to us. As part of the Program, Silver House complies with the Health Insurance Portability and Accountability Act (HIPAA) to protect the confidentiality and security of your personal health information (PHI). By participating in the Program, you acknowledge that Silver House will collect, use, and share your health information in accordance with HIPAA regulations. For more detailed information about how we handle your PHI, please refer to our Community HIPAA Privacy Policy.

  1. Program-Specific Terms

The Program incorporates a variety of programs, each with its own set of terms and conditions. By enrolling in the Program, you may be subject to the specific terms associated with individual programs like BATWatch™, Circles™, and CogStim™. Please review those respective terms.

  1. References to External Providers

Certain Program services, such as diagnostic testing and remote monitoring, may involve third-party providers. These external providers are governed by their own terms and privacy policies, which you agree to by utilizing their services. You can access their specific terms of service and privacy policies via links provided within the service portals or through our referral agreements.

  1. Digital Signature Consent for Streamlined Lab Services

In addition to the consents provided above, we want to ensure that you fully understand how your digital signature and personal information will be used as part of the Program. Please review the following authorization carefully.
Specifically, you acknowledge and agree to the following:

15.1 Digital Signature Use: Your digital signature, as captured on this consent form, may be used to sign other required consent forms on your behalf, including but not limited to forms for third-party laboratories, providers, and partner labs within our Collaborative Care Community.

15.2 Authorization for Partnered Labs and Providers: You give Silver House express authorization to sign additional lab consent forms and other documentation required by our partnered healthcare providers or labs within the Collaborative Care Community. This authorization helps streamline the process for ordering tests and other services on your behalf, ensuring that your care can be coordinated without unnecessary delays.

15.3 Form Variability: You understand that consent forms required by different laboratories and service providers may vary in format and structure. By authorizing Silver House to affix your digital signature, you agree to allow us to handle these variations, ensuring that all necessary documentation is completed efficiently.

15.4 Handling of Irrelevant Information: Silver House is committed to protecting your privacy and ensuring that only relevant and necessary information is provided to third parties. Specifically, Silver House will not disclose any demographic information, such as race, ethnicity, or other unrelated questions (e.g., additional diagnosis codes irrelevant to the lab order requested) as asked on forms from specific lab providers, or similar providers. This methodology applies to all irrelevant questions on forms, and Silver House will not fill out such sections on your behalf without your specific, explicit authorization.

15.5 Disclosure of Financial Aid Information: The only exception to the above is the disclosure of relevant financial aid or assistance details, which may be necessary to reduce or waive any fees associated with the ordered testing. This information will be shared solely for the purpose of facilitating financial assistance and ensuring that you receive the necessary care without undue financial burden.

15.6 Purpose and Transparency: The purpose of this process is to reduce the administrative burden on you as a participant in the Program. Silver House aims to create a seamless experience by handling the complexities of form completion while safeguarding your personal information and ensuring that only necessary data is shared.

15.7 Accuracy and Integrity: Silver House will take all necessary steps to ensure that the information submitted on your behalf, using your digital signature, is accurate and consistent with the information you have provided. This includes verifying that each form is correctly filled out according to the requirements of the relevant laboratory or service provider, while omitting any irrelevant or unnecessary information.

15.8 Right to Withdraw: You retain the right to withdraw this authorization for the use of your digital signature at any time by submitting a written request to Silver House. Any actions taken prior to the receipt and processing of your withdrawal request will remain valid.

  1. Grouped Service Orders for Streamlined Coordination

As part of our commitment to making your experience with the Collaborative Care Program as convenient as possible, we may group service orders, such as blood draw requests, to streamline coordination. Please carefully review the following authorization regarding this process.

By agreeing to this policy, you acknowledge and consent to the following:

16.1. Grouping of Service Orders:

To facilitate convenience for participants, particularly for family members, friends, or residents in assisted living facilities, Silver House may submit grouped service orders for blood draws or other healthcare services. This means that, where possible, requests for multiple individuals may be combined into a single communication or order to streamline coordination and scheduling with laboratories or similar providers.

16.2. Secure Communication:

All group orders will be submitted via secure, HIPAA-compliant email systems to ensure the protection of your personal health information. Silver House is committed to safeguarding your privacy while offering efficient and coordinated care.

16.3. Coordination of Care:

By grouping service requests, Silver House aims to reduce logistical complexity and minimize wait times for participants who prefer to receive services together. This includes individuals living in shared housing arrangements or families who wish to schedule services simultaneously for added convenience.
